Hospice Nurse

Advanced Hospice has IMMEDIATE OPENINGS for hospice nurses. If you have a passion for hospice this is an excellent opportunity to work with a great TEAM. We are hiring both full and part-time nurses.

RESPONSIBILITIES
• Collaborate with physicians and other professionals to plan care
• Make frequent visits to the patient to evaluate their condition
• Provide support and comfort according to individual patient’s needs
• Work with caregivers to plan and execute excellent patient care
• Assist patients and their families
• Maintain accurate records

 

SKILLS
• Knowledge of nursing care processes and medication guidelines/side-effects
• Committed to safety precautions and high ethical standards
• Sound judgement and attention to detail
• Excellent communication skills and a compassionate nature
• Formal nurse training: certification as hospice nurse is a plus
• Current CPR certificate

 

EDUCATION Licensed LPN/RN or BSN with current PA state license.
EXPERIENCE Hospice experience preferred but not required.

Ability to work independently and within a multidisciplinary team. Valid driver’s license necessary.

Hospice CNA

If you are an experienced hospice aide who is compassionate and possess excellent interpersonal skills, we want to hear from you!

 

DUTIES OF POSITION
The hospice aide is responsible for providing personal care under the direction of a registered nurse.

 

RESPONSIBILITIES
• Personal care (bathing, dressing, toileting, feeding, etc.) according to the
Hospice Aide Plan of Care.
• Document care provided according to agency policy.
• Report any deviations from the Plan of Care Perform to the Case Manager (or designee if not available).
• Use Durable Medical Equipment safely.
• Take vital signs when on Hospice Aide Plan of Care.
• Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
• Provide proper care and observation of patient's skin to prevent breakdown
• Assist with ambulation, transfers, range of motion if documented on the Hospice Aide Plan of Care.
• Participate in QAPI activities.
• Participate in IDG meetings, as appropriate.

 

QUALIFICATIONS

1. Meets training requirements of the State.
2. Received a minimum of 75 hours of classroom and supervised practical training with at least 16 hours devoted to supervises practical training and with at least 16 hours of classroom instruction completed.
3. Two years experience as a CNA, hospice aide or home health aide. (CNA REQUIRED)
4. At least 18 years old.
5. Must be a licensed driver with an insured automobile in good working order.
6. The ability to communicate well, both verbally and in writing.

Hospice Volunteer

Volunteers are needed to share their time and talents for:

• Clerical tasks

• Fundraising events

• Educational Fairs

• Patient Care visitation

• Bereavement support

 

Training provided relevant to tasks being performed.  Volunteering with Advanced Hospice is sure to be a most rewarding experience!
